Baptism Intention Form

Following the Lord in baptism is an act of obedience and a physical representation of a spiritual reality. This form is meant to help us better understand your story and help you articulate how it fits into the greater gospel narrative. Please take some time to prayerfully answer the following questions. 

Testimony

At Four Oaks Midtown we value stories of redemption, and your story speaks to the goodness of Jesus. Therefore, when you are baptized at Four Oaks Midtown, we ask that you to share a 3-5 minute testimony to those gathered as witnesses. The questions below serve as a template for writing your testimony. Consider answering a few questions from each section below in a “life story” format to help clarify your thoughts. It should read more like you are telling a story than bullet points and should be right around 400 words long (or whatever length reads at 3-5 minutes). 

Children seeking to be baptized have a story, too. Often, this story revolves around God's gracious keeping power. Children also wrestle with sin and temptation, and their story may speak to how God has kept them, how Jesus intercedes for them, and how the Holy Spirit empowers them to live a life pleasing to God. Parents, please help your children think through these pieces of their story, as theirs is also a story of God's redeeming grace. 

Baptism

Baptism is an ordinance of the New Testament, modeled (Matthew 3:13–17) and ordained (Matthew 28:18–20) by Jesus Christ, to be to the person who is baptized -

a sign of his fellowship with Christ in His death and resurrection (Romans 6:3–4)

a sign of his being engrafted into Christ; of remission of sins (Galatians 3:27)

a sign of that person's giving up of himself to God, through Jesus Christ, to live and walk in newness of life (Acts 2:38, 41)
